I'm currently in-network with: Aetna and Premera. I'm in the process of credentialling with First Choice and Regence.

I am considered an out-of-network provider for all other insurance plans, including Medicare and Medicaid.
If I am out-of-network for your insurance, please contact me for current rates for initial assessments and follow up visits. If your plan offers out-of-network benefits, I am happy to provide you with a superbill that you can submit to your insurance company for potential reimbursement, according to your benefits.

I do not offer case management services, disability or forensic evaluations, court mandated assessments and/or treatments, Labor and Industries (L&I) evaluations, evaluations of capacity or fitness for employment, firearm/weapon ownership, or child custody evaluations.
